A Program for Home Care Patients
Health Care Partners (HCP) is a state certified Medicare agency. We deliver skilled professional medical care in the privacy of the patients home. By law, a home health care provider is the choice of the patient. To qualify for Medicare part A benefits, the following criteria must be met:
Patient is homebound.
Patient is currently under the care of a physician.
Services are reasonable and necessary.
Services are intermittent.

The term “HOMEBOUND” means a condition due to illness or injury which restricts a patient’s ability to leave home without assistance. Absences are infrequent and of short duration.


Skilled Nurse

  • Post-op care with teaching
  • Skilled observation and assessment
  • Wound care
  • Foley catheter insertion and care
  • Diabetic care and education
  • Medication management
  • Bowel and bladder training
  • Ostomy care
  • NG tube care
  • IV therapy
  • Evaluate and treat infections
  • Teaching for any disease process

Physical Therapy

  • Home safety assessment
  • Pain management
  • Gait/Mobility/Transfer Training
  • Muscle strengthening

Occupational Therapy

  • Activities of daily living training
  • Fine motor skill training
  • Assistive device training

Medical Social Worker

  • Assessment of social and emotional well being
  • Counseling for short and long-term planning
  • Community resource planning

Speech Therapy

  • Restorative speech language pathology services
  • Treatment of dysphasia-swallowing problems
  • Post CVA Rehabilitation

Home Health Aide

  • Assistance with personal care
  • Complete or partial bed bath
