Program for Home Care Patients |

| Health Care Partners (HCP) is a state certified
Medicare agency. We deliver skilled professional medical care in the privacy
of the patients home. By law, a home health care provider is the choice
of the patient. To qualify for Medicare part A benefits, the following criteria
must be met: |
Patient is homebound.
Patient is currently under the care of a physician.
Services are reasonable and necessary.
Services are intermittent. |
The term “HOMEBOUND” means
a condition due to illness or injury which restricts a patient’s
ability to leave home without assistance. Absences are infrequent and
of short duration.
