SPECIALTY CARDIOVASCULAR PROGRAM

Health Care Partners (HCP) has a home-based Cardiovascular Program designed to work with patients who have had a decline in function following cardiovascular surgery.

This program is a multi-disciplinary approach with an emphasis on returning each patient to a level of function equivalent to or better than what was maintained prior to hospitalization.

The plan of care is individualized to each patient and is a collaborative effort from professionals to ensure quality of care.


Intense Therapy
HCP offers all post-operative patients an individualized exercise program in the home.

INTENSIVE THERAPY BENEFITS:

  • Physical therapy five of the first seven days after discharging from the hospital
  • Teaching to identify cardiac precautions during mobility
  • Awareness of upper body ROM restrictions
  • Reducing post operative complications through exercise


The main objective for physical therapy while recovering from cardiovascular surgery is to gradually and safely recondition the heart and body by increasing the amount of time you are able to exercise.

SERVICES AVAILABLE TO PATINTS:

  • On call professional services seven days a week – 24 hours a day
  • An individualized home based recovery plan to safely recondition the heart and body
  • Skilled Nursing to monitor status of surgical wounds, vital signs, SpO2 & PT/INR’s ensuring safe quick recovery
  • Education on risk factor modification – controlling cholesterol, blood pressure and weight
  • Teaching to identify signs and symptoms of complications
  • An individualized plan of care to ensure a successful surgical outcome

Health Care Partners Specialty Cardiovascular Program will assess each patient’s status at the time of hospital discharge to set a baseline. HCP will evaluate and manage the effectiveness and outcome the program has on post-surgical patients.

Health Care Partners will provide a transition period for you to recieve physical therapy in your home until your surgeon agrees you are ready to participate in an out patient therapy setting.

Exercise
THE PRIMARY STEP TO A SUCCESSFUL RECOVERY

An exercise regimen for the cardiovascular patient is designed to increase the heart’s efficiency, lower blood pressure, increase strength and flexibility to improve balance and reduce falls, decrease stress, and create an overall improvement in the quality of life.


Mobility
ESSENTIAL TO AN ACTIVE LIFE

Our staff helps you through the post-surgical process. We provide instruction on energy conservation, cardiac precautions, DME, and adaptive devices. Emphasis will be placed on safety as you perform the tasks required to care for yourself.


Pain Management
THE KEY TO ACTIVE PARTICIPATION DURING REHAB

Pain inhibits your ability to fully participate in physical therapy over the course of a four to eight week recovery.

HCP staff will provide education to help decrease pain and improve your activities of daily living. Your physician and physical therapist will work with you to develop a plan consisting of prescribed medication and other methods necessary to relieve pain.


Education
AN EDUCATION PATIENT IS A HEALTHY PATIENT

It is critical for the patient to play an active role in the healing process. HCP will guide and assist you in the recognition of post-operative complications. A successful recovery involves a well informed patient.
